摘要(Abstract)：

“碎片化”的医保体系是阻碍县域医疗卫生资源实现均等化的重要因素。本文通过梳理现有研究发现，当前医保整合存在对居民就医引导不足、群体间受益不均、区域间收益不均的问题，难以发挥对县域医疗卫生资源配置的引导作用。笔者以机会平等理论为基础，提出提高全民医保待遇水平与县域区域分级诊疗相结合的医保整合建议。最后，运用合成控制法，分析机会平等理论视角下医保整合引导县域医疗卫生资源配置的作用。结果表明，整合能够提升县域层面每万人的病床数和医疗技术人员数，但对提升县域层面每万人的医疗机构数作用不显著，且存在潜在的负向影响。同时，政策的持续性不足。因此，未来医保整合应在关注医保弱势群体的同时，注重县域区域就医的引导，并优化激励制度，提升转诊服务水平。
